PREPARING FOR APPOINTMENTS FOR IN-PERSON / TELEPHONE / VIDEO COUNSELING
We ask that prospective clients complete our Registration Form to give us the information we will need to verify participation in your health plan and our ability to accommodate your scheduling needs. Expedited and efficient access to our services is best obtained by completing the following 4 steps:
Once completed, make a copy for yourself (before submitting), if possible, and then hit the “Submit” button. All the information will be transferred to our office, and you should receive an emailed confirmation that we received it within 24-48 hours.
Note: If you are unable to–or do not wish to–complete and submit this form to us using this method, or you are having difficulty, you may call our office at 360-626-1457 to provide the necessary information and request an appointment. When the counselor is not in the office or is with clients, please leave your confidential message.
If, before your visit, you do not want to read the documents in Step 2 or complete the form in Step 3, you may skip to Step 4 and read/and or complete forms at your first visit. (Note that for telephone and video counseling, Steps 2 and 3 are required in advance.)
STEP 2: Once you have set up an appointment, every participant in the counseling is asked to please click on each title to read each of the following documents:
STEP 3: Select one of the following two links related to the mode of counseling you are requesting. Then complete and submit the form to attest that you have reviewed the above forms and affirm that you understand and agree to the policies and procedures specified. (Every participant–adults and children 13 or older–must complete and submit one of these forms.)
For COUPLES COUNSELING, both parties also please complete this form:

STEP 4: Before going to your first appointment, check your email for a confirmation of the date and time and other important information.
